Medical Case: Dermatologist Removes Massive, Long-Standing Blackhead from Patient

A recent dermatological procedure has drawn attention after a video documented the extraction of a large, deeply embedded blackhead from a patient’s back. The procedure, performed by a licensed medical provider, involved numbing the area before carefully removing a dry, keratin-filled plug described as feeling like a “pebble” under the skin.

The blackhead, formed over years of accumulated dead skin and sebum, had oxidized at the surface, giving it a characteristic dark appearance. Using sterile tools, the practitioner made a small incision and extracted the compacted material in segments, including a solid “black pearl”–like core.

Such cases, while not common, are treatable through professional care. Dermatologists warn against at-home extraction, which can lead to infection, scarring, or pushing the blockage deeper into the skin.

Patients with persistent or large blackheads are advised to seek medical evaluation rather than attempting self-removal.
